Medical Forms

The Department of Intercollegiate Athletics wishes to welcome you as a sports camp participant. Every camp carries with it some degree of risk to the participant because of the physical nature of the activities. The UT Sports Medicine Staff in cooperation with the University Health Services and a network of consulting physicians and certified athletic trainers provides specialized services, care, and supervision to safeguard your health and well being.

Each camper’s parent or legal guardian must complete the required medical forms prior to any camp participation.

** All Medical Forms are due by June 1st at 5pm **

Note: A physician’s signature is required on one of these forms; however, the participant’s current school physical may be attached in lieu of a doctor’s signature (only if performed within a year of the camp). The medical forms must be submitted prior to the start of camp, no forms will be accepted on site.

Each participant must be covered by his legal guardian’s insurance before participating. Medical aid is provided on site by a licensed athletics trainer or the Texas Sports & Family Medicine or a local hospital. Campers’ parents/guardians are billed directly for all medical services rendered by Texas Sports & Family Medicine or a local hospital, and not covered by their insurance.
